Spots on the Penis

Small red spots that show up after shaving are almost always razor burn or folliculitis — irritated hair follicles, sometimes with a tiny bump at the center of each one. They're usually tender to the touch and cluster in the area that was shaved. A red, flat rash that's spreading rather than staying in a few discrete spots is more often a reaction to soap, detergent, or friction than an infection.

Red spots that are genuinely new to you, especially in a ring or cluster, and especially if they came with any tingling or burning first, are worth taking more seriously — that pattern overlaps with early herpes.

Small, painless white or pale-yellow bumps clustered on the shaft are frequently Fordyce spots — enlarged oil glands, completely normal, not contagious. Genital warts can also appear pale or flesh-colored, but they usually have some texture to them rather than sitting flat, and they tend to grow gradually over weeks rather than appearing all at once.

A single dark spot that's been there for a while and isn't changing is usually just a benign skin variation, similar to a freckle elsewhere on the body. What's worth flagging to a doctor is any spot that's changing shape, size, or color, since that's true of skin anywhere on the body, not just here.
