Sore Throat

Realistically, a sore throat is a cold or a mild virus the overwhelming majority of the time. But oral sex can transmit a handful of STDs to the throat, and because those infections often look and feel exactly like an ordinary sore throat, they get missed constantly — mostly because nobody thinks to ask for an STD test when they assume they just have a cold.

Why this is easy to miss

Oral (pharyngeal) gonorrhea very often causes no symptoms at all. When it does, it tends to look like a fairly unremarkable sore throat — some redness, some discomfort swallowing — nothing that would make you think "STD" instead of "cold." Oral chlamydia behaves similarly: mild, easy to write off, sometimes accompanied by a bit of hoarseness.

Syphilis is a little different — during its early stage it can cause a painless sore in the mouth or throat, which people frequently don't even notice because it doesn't hurt.

What actually separates this from a regular cold

Timing and context do the heavy lifting here far more than the symptom itself. A sore throat that shows up within a week or two of oral sex with a new or non-monogamous partner is worth considering separately from a sore throat that shows up alongside a runny nose and cough during cold season. A sore throat that lingers well past the usual week a cold takes to clear is also worth a second look, regardless of how it started.

None of the throat-based STDs are reliably detectable through a standard swab used for strep — testing for them uses a different, specific swab, which is part of why they're so commonly missed in a routine urgent-care visit unless you specifically mention recent oral sex.

When It's Worth Testing

If a sore throat follows oral sex with a new partner, or simply doesn't resolve in the usual week or so, it's worth testing for throat-based gonorrhea and chlamydia specifically — a standard strep test won't catch these.
