Rash on the Penis
A rash — a broader, less discrete change than a single bump or spot — on the penis most often comes from irritation or a fungal infection rather than an STD, but it's worth knowing the exceptions.
The common, non-STD causes
Contact dermatitis from soap, laundry detergent, or a new fabric is one of the most common causes, and it's usually red, sometimes itchy, and limited to wherever the irritant made contact. Jock itch (a fungal infection related to athlete's foot) causes a red, sometimes scaly rash, often with a well-defined edge, and thrives in warm, damp conditions. Balanitis — inflammation of the head of the penis, often related to hygiene, a mild irritation, or a yeast overgrowth — causes redness and sometimes swelling, more common in uncircumcised men.
When it might be an STD
Secondary syphilis can cause a rash that spreads beyond the genitals, notably to the palms and soles — a pattern that helps distinguish it from a localized irritation. Herpes causes clusters of blisters rather than a diffuse rash, which typically progress to open sores. A rash that's spreading, recurring, or accompanied by other symptoms (fever, swollen glands, discharge) is a different situation than a one-off irritation that clears with basic care.
When It's Worth Testing
A rash that doesn't improve with basic hygiene and avoiding the likely irritant within a week or two, or that comes with other symptoms, is worth testing for rather than continuing to guess.
