Lump on the Testicle
Of everything on this site, this is the one topic where the honest advice is: don't just read about it, go get it looked at. Most testicular lumps have nothing to do with an STD — but a small number of causes, including testicular cancer, are genuinely time-sensitive, and the only way to tell the difference is a physical exam.
The common, non-urgent causes
A varicocele — enlarged veins in the scrotum, similar to varicose veins — is one of the most common causes of a lump or fullness, often described as feeling like "a bag of worms" above the testicle. A hydrocele is a fluid-filled sac that causes painless swelling and is common enough that it's rarely anything to panic about. Epididymal cysts are small, usually painless lumps on the coiled tube behind the testicle, and they're extremely common as well.
Where STDs come in
Untreated chlamydia or gonorrhea can occasionally lead to epididymitis — inflammation of that same coiled tube — which shows up as swelling, tenderness, and sometimes a lump, often alongside pain when urinating or discharge. This is treatable with antibiotics once identified, which is exactly why testing (not guessing) matters here: the fix is usually straightforward, but only once you know what you're dealing with.
Why this one gets a doctor visit, not just a test
An STD test can rule chlamydia and gonorrhea in or out, but it can't evaluate the lump itself. A hard, painless lump is the classic early description of testicular cancer — which, caught early, has an excellent outcome, and caught late, is a much harder conversation. That combination (common, sometimes serious, and highly treatable if caught in time) is exactly why any new testicular lump deserves an in-person exam rather than a wait-and-see approach.
When It's Worth Testing
Get an STD test if the lump comes with pain, swelling, discharge, or burning during urination. But also — separately, and regardless of the test result — get the lump itself examined by a doctor. Testing and a physical exam answer different questions.
